Option 1: Payment in Full at Time of Service
We accept the following payment methods:
- Cash
- Check
- Credit Card – Visa, MasterCard, Discover
A processing fee applies: 3% for major credit cards and 2% for debit and HSA payments. (Effective 1-6-2026)

Option 2: Insurance and Partial Payment
When time permits (3–4 weeks), we can submit a pre-determination request to your insurance company to provide an estimate of your out-of-pocket costs at the time of service.
If this is not possible due to time constraints, 30% of the total fees will be due when services are rendered.
After your insurance has processed payment, any remaining balance will be your responsibility.

Option 3: Third-Party Financing
Patients may qualify for monthly payment plans through CareCredit. You can apply online at www.carecredit.com or by phone at (800) 365-8295.
Any fees not covered by insurance or paid at the time of service can be financed through an active CareCredit account.
As a courtesy, we will file insurance claims using the information you provide. Because our services are rendered directly to you, you are responsible for all fees incurred. By signing a separate document, you authorize NorthernStar Oral & Maxillofacial Surgery, PLLC to receive any insurance benefits that would otherwise be payable to you.
